Guest / Friend / Member Information Form
New to Wesley or want to update your info? Submit your name, address, phone, email, household members, birthdates, and anniversary to the church office. Can be filled out in person or emailed to church@ishpemingwesley.org.
Action Request Form
Use this form to submit a proposal or request for action to the Leadership Board. Covers the purpose, issue/proposal, rationale, anticipated cost, and funding implications. Submit to the church office at least 10 days prior to the monthly leadership board meeting — held the 3rd Tuesday of each month.
New Ministry / Project Planning & Evaluation Form
Proposing a new ministry or project at Wesley? This form covers project description, team members, expected outcomes, budget, and advertising plans. Includes an evaluation section to complete when the ministry concludes and reflection questions for future planning. Return to the church office for pastor and ministry coordinator review.

Youth Medical Release & Permission Form
Required for all youth participating in Wesley youth group activities including Breakfast Club, Youth Group, and High School Life Group. Covers medical history, allergies, insurance information, emergency contacts, physician and dentist info, photo permission, student conduct agreement, and parental consent and liability release for all church-sponsored activities.
Policy for the Protection of Children, Youth & Vulnerable Adults
Wesley UMC's official child protection policy, adopted July 9, 2024, aligned with the Michigan Conference UMC policy. Covers Safe Gatherings screening and background check requirements, the Rule of Three supervision standard, volunteer qualifications, mandatory reporting procedures for suspected abuse or neglect, online ministry guidelines, and follow-up protocols. Required reading for all adult leaders, volunteers, and staff working with children, youth, or vulnerable adults.
Sunday School Registration Form
Register your child for Sunday School at Wesley UMC. Includes child's name, date of birth, grade, allergies/special needs, parent/guardian contact information, emergency contact, and photo permission. Return completed forms to Sunday School, via email, or to the church office. Contact Christian Education Coordinator Pam Olgren for more information.

Endowment Fund Grant Application
Apply for funding from Wesley's Endowment Fund for ministry projects and programs. Deadlines are March 1st and September 1st each year.
